• Page of observations
• ECR/Non-ECR page
• For a passport with short validity, a validity page must be submitted
• A death certificate or divorce order (if the spouse's name has changed)

The process to change a spouse's name on a passport remains the same. If necessary, the applicant must provide either the death certificate or divorce decision for the former spouse listed on the passport.

How to Add Your Spouse's Name to Your Indian Passport?

Adding your spouse's name to your passport follows the same process as having your passport reissued. Hence, there are 2 ways to get your passport reissued.

1. Apply Online

Follow the steps below to submit an online form.

• Visit the official Passport Seva website and register yourself.
• After logging in using your ID and password, choose the option "Apply for Passport Reissue" to add your spouse's name to the passport.
• Click on "Submit" after entering all the necessary information.
• To make an appointment, choose the "Pay and Schedule Appointment" option.
• You will now be transferred to the "Payment" page. Here, choose your preferred payment mode (such as debit/credit card, online banking or SBI challan).
• Select the "Print Application Receipt" option.
• This receipt will include the number of appointments and the application's reference number.

Note: Appointments at POPSK/PSK require you to pay the online fees beforehand.

2. Apply Using e-Form (Offline XML)

Follow the steps below to submit the e-form Offline:

• Access the official Passport Seva Kendra website.
• Download the e-Form in XML format. 
• Since the application is being filled out offline, you must upload the data in XML format to the system.
• Fill in the form with all the necessary details.
• If you are adding your spouse's name, choose the "Reissue of passport" option. Afterwards, tick the spouse's checkbox under "Change in existing personal particulars".
• Log in to the website and upload the XML file.
• Pay the applicable fees using your preferred payment mode.
• Download the payment receipt for future reference.

Note: Payment must be made online via credit/debit cards, netbanking or UPI.

Passport Reissue Fees for Adding a Spouse's Name

To add your spouse's name to your Indian passport, you need to apply for a passport reissue under the "Change in Existing Personal Particulars" category. The fee depends on the passport booklet you choose and whether you apply under the Normal or Tatkaal scheme.

Note: The Tatkaal fee is charged in addition to the normal application fee. The final amount payable depends on the type of passport booklet selected and the service requested.
