What is Annexure C for Passport Applications?

It is a necessary declaration required in India when applying for a minor's passport. Under Section 5 of the Passports Act, 1967 and the Indian Passport Rules, 1980, Annexure C ensures that a single parent can provide consent for a minor's passport. The form can be used to explain why the other parent did not sign, including being overseas, a lack of cooperation, custody disputes or parental neglect. The form includes the minor's full details like name, date of birth, citizenship and the identity proof of the signing parent.  

When is the Annexure C form required?

If an individual is a single father or mother responsible for supporting all their children, then they are responsible for submitting the Annexure C form. 

  • Untraceable Parent

If one parent's location is unknown and the other parent has not made the necessary effort to locate the absent parent, then the latter must provide the Annexure C form for the passport. 

  • Legal Guardianship Situations

Annexure C for the passport and court documents that verify you are a legal guardian must be submitted. 

  • Child Born out of Wedlock

If a child was born before the parents' marriage and is being raised by a single parent. 

  •  Separated but not Divorced Parents

Annexure C for the passport is required if the parents are not living together and are also not divorced. It is necessary to clarify the circumstances and to progress the application without consent from the second parent.
