You can re-issue a damaged OCI card by filling out an application form online at the OCI services website> apply with the relevant supporting documents, photograph, and signature> submit the original supporting document to the Indian mission/post-FRRO for verification purposes> go for a personal interview at the date and time given by the concerned Indian mission/ post office.

I have lost my OCI card for which I have to give an interview for a new OCI card application. When I submitted my application I received the time and date of the interview.
The documents I gave in my OCI card reissue are a damaged OCI card, proof of address, and a copy of my new passport.
